Most people (that have a capable PC) are using an offline local setup of Stable Diffusion on a WebUI.
However, before using SD, I would recommend starting with something like this to get a basic feel for how AI image generation works. It's completely free and can make some pretty cool stuff. And yes it can easily make NSFW stuff.

Once you feel that you have a basic understanding of how stuff works and If you want to be more serious about creating AI images/videos, follow one of these guides to install one of these WebUIs.
HOWEVER, Don't get your hopes up until you check the PC spec requirements. If your PC meets the specs than proceed.
Automatic1111 (recommend you start here)

Forge WebUI (Use this once you get a good feel for A1111)

ComfyUI (Only use this once you have a good understanding of both Forge and A1111)
You must be registered to see the links
Finally for everything you need, Like checkpoints, Loras, etc. go here. (I recommend starting with SD1.5)

It's all free and open source. If you come across something that has a price tag it is bullshit.
