Most of the challenge in non-VNs is struggling with bad UI.
But some possible options:
Wife Trainer Files - Legendary game. Great writing, great buildup, not your normal netori since most of the time the husbands are sending their wives to you (except for a few regular cheating scenes). Poses a decent challenge, since you only have a limited time to train each wife to achieve the desired results and if you fail, they're gone for good.
Glassix - Damn, this game is over 6 years old and still getting updates! Dated visuals (they weren't great even when it first came out), but huge cast of characters and plenty of them of the netori persuasion. Utako's story is especially tasty. The challenge is mostly how you manage your time exploring the city and developing the relationships, so there's a fair bit of grind. I usually cheat my way through those, but others may prefer to do it oldschool.
Lab Rats 2 - Mostly procedural content, both relationships and models, so it's missing the personalised feel. But if you can abide that, there's a good chunk of scenarios that are devoted to infidelity. NPCs that are cheating on their SOs with you will have different dialog and scenes (including some where their guy walks in and catches you in the act). Not my cuppa, because it takes too much time setting up a scenario with enough characters that appeal, and the payoff isn't worth it imo. Not a fan of the management meta game either, even though it poses a decent enough challenge for someone playing it the first time.
New Coral City - Alright, this one might be more a VN, but you want a challenge, good luck keeping track of all the relationships out here and who gets pissed when they find you're bonking whom.
Love, Lust, and Rock 'n Roll - Combo of stat builder, with conversation checks and relationship buffing. Not the most elegant system, but I'm just trying to hurdle it so I can get to those really well-crafted scenes.
If RPGM games count:
Urban Demons - Still the baddest shoto milf-busting sim out there. Occasionally buggy, but can be played through to completion. Wish it was on a platform that was easier to mod, RPGM is a royal pain in the ass.
Demon Deals - Sort of a spiritual successor to Urban Demons with improved art. Don't get me wrong, the Glassfish characters have been immortalised, but the illustrations here are halfway between Summertime Saga and Urban Demons in the right way.
Have probably forgotten others, will update if I remember some more.

The King of Summer [No Try Studios] - Kendo gets some nice progress with one of his conquests.
Taking some time off from his new girls, Kendo goes back to Natsuki and asks her to spend some time with him. She declines, saying she wants some time with her husband Yoshi. Last update ended with you staying over at Meredith and her husband Tommy's house, after you protected them from a thug attack. You meet her in the bathroom in the middle of the night, where you persuade her to suck you off as practice for the next shoot. The new update begins with you invading her morning dream, in which she's having sex with you, but preventing herself from reaching orgasm. You take control and break through the barricade quite convincingly.
Julia comes over and shepherds all 3 of you to an exclusive hotel (a demonic one, but the couple don't know that) where you'll be safe from further attacks. The plan is to do the next shoot there, so Meredith and Tommy are told to refrain from having sex, so she can be fresh for the shoot.

Frustration [J&R Games] - Game starts off like it's going to have some good eatin' for netori fans, but pulls the rug after a tease. Bah! Still, seems like a good game with some fun writing. Some grind added for no good reason, but can be tolerated.
MC is a servant of a succubus who has been awakened in the present day to serve again. His specialisation is to collect mana for his mistress by fucking women, and it takes him a bit of a learning curve to get used to modern hookups, but then he's ready to go.
One of the women who catches his eye is a housewife Marie who he sees on the balcony across the street. At first, his perving causes her to panic, but later on, she gets into it, starting to flash him. He takes the initiative, visiting her house, pretending to be a plumber to her husband. She's nervous, but plays along, letting him grope her behind her husband's back, and finish by cumming over her bare tits.
Sadly that ends the netori phase. First, she backs off, saying it was wrong. Then you get evidence that her hubby is visiting a hooker on the side, and you expose his whole operation. OnlyWife: Hunting sleeping [sunset] - This should have been an ideal game for this thread in principle. But the borderline incomprehensible translation, and the amateurish setup in general makes it one big fat disappointment. Doesn't really deserve any spoiler text.

The King of Summer - Fast updates, lots of netori, good writing. Might have to put up with a fat MC, the Koikatsu anime style and some excessive wordiness.
Our Red String - Great netori buildup with Minerva and Cindy. A caveat, this has 2 protagonists, one male and one female. They can get into a relationship, stay friends with benefits, cheat on each other, etc. Shouldn't be a deal-breaker.
Sisterly Lust - Granted, this is mostly incest, but there's a great subplot with the preacher's wife Iris. Be sure to download this extra scenes version, rather than the one linked in the OP.

Also added some dirty talk during sex based around her being married. Have tried to avoid going over the top and keep it natural.
