The last time I played this was i 2021... Could someone remind me of certain things?
-With the lewd patch the beginning of the game is totally different, right?
-Even with ntr on and choosing ntr options, the game makes me replay the scene and choose the non-ntr option, is it supposed to be like this?

Patch restores the game as it was "intended" to be played. Without it, it becomes overly safe to comply with patreon standards.
The NTR is just a bad end, to show the consequences if Hunter ever succeeded. It's not a route, but just a glimpse. You need to make the Vanilla choice to proceed with the canon path.
