Some people just use these games as porn. They care don't care who fucks whom because they aren't invested in the characters' relationships. That means the main point of NTR is irrelevant to them. Possibly all context is irrelevant to them. They just want to see hot sex, and they want to see variety, and NTR games coincidentally meet those criteria.
Now, one might say that such indifference to NTR could mean they lack empathy, but I'd disagree. The tendency to compartmentalize sex and emotion is very common, especially if our emotional needs are being met some other way, then sex can be JUST sex.
It clearly wasn't meant as an attack on people who ARE emotionally invested, so there's no need to be defensive. There's also no need to present everything as a false dichotomy i.e. "if you don't think x, that means you think y!" Sometimes others' perspectives don't fit into our preconceived framing.

And to add to it, the main characters are usually so badly written that you want another character to sweep in and steal the girls, not to mention that the "good" routes usually have the worst sex scenes when compared to their counterparts.
Example: Melinda from FoTM is a character that never got fucked properly since the mc was a whiny self entitled crybaby and it literally ruined the game. The dev tried adding NTR at one point but the outrage was so big he scrapped it and then the game ended up dying shortly after.
Maybe i'm in the minority but some of us just want to watch our favorite female characters get fucked by something other then a whiny mc.

There also seems to be some kind of an incel civil-war element on either side.
