Keep refusing them, and dropping your reputation as much as possible. You also need -300 rep. And maybe a random chance for the event to trigger. I have gotten that ending, so it works.

It's more just impregnation content right now, with pregnancy as an ending. Tomie automatically gets an abortion after being knocked up, except for the ending triggers.

Could you add some repeatable traits? Sometimes I have a couple trait point left over at the start, and it would be nice to spend those on things like starting cash, skill points, or stat boosts (maybe increasing max willpower, etc?).
Some of the scenes, like the hospital volunteer job, and the nightclub require a lot of clicking. Could you make it so holding the mouse button also works to advance the text? I do like the hand groping effect in those scenes, it really sets the feeling of Tomie being felt up and molested.

In the case of automatic progress with the mouse, press the SKIP or AUTO button to proceed (keyboard shortcut L-Ctrl or A key).
