Extract the downloaded file with winrar or winzip. You will get a file called unren.bat, copy that file and paste it inside the game folder of the game you want, then open unren.bat there and type 1 in the keyboard, to extract the rpa images. Then you get a folder called 'images' inside the game folder.

What does the story have to do with the development pace. It is L&P who is on the slower side and his story is massive. And anything that is done by a single person with emphasis to quality takes it's time.
1.5 hrs of a Disney movie takes around 5 years to make even with a 100 animators. Anime generally having 12 episodes ( 4 hours ) generally take 10 months to one year in production, sometimes even that is not enough. Some extremely mesmerizing scenes which last only 2-3 minutes take 3-4 months to make. A manga chapter not containing more than 10-15 minutes of reading time takes 1 month, some with higher quality art take 2-3 months for 10-15 minutes of content.
The story itself can be told in very less time if done through text or shot with real people, when it has to be drawn or animated it takes time to get all the details right. So good storys can take a little extra time but the real time taker is the length of the story, the medium chosen to narrate it,the number of people behind it and the quality at the which the product is intended to be delivered.

First of all, this isn't a movie or animation, and to compare this to a film is like comparing apples and oranges. I never said it doesn't take time; I said again, "A good story doesn't have to take 20+ years to tell either." Second, A good story doesn't need complex storylines either. A direct, simplistic approach can work just as well. Other VN novels are good at the core and are linear, which don't branch out into 30+ different paths. He could have written the story based on six or seven characters and most likely achieved his results at a quicker pace.
If you read some of his Patreon comments, most want to see the core characters with Sophia more than anything else, Dylan, Ellie, Patricia, and Sam, plus some outside characters like Aiden. If he focuses on them more instead of having 90+ characters, I think he would achieve more significant progress with less, and his Patreon Member count would be higher than it is now.
He could have done this in less time if he didn't have, me being facetious here, 30+ routes with some of the routes having the same themes as others.

There's no real answer to that, since it depends on what you find interesting. To play all the scenes I think most have about 6 or so. But of saves for the "interesting scenes" some have dozens of saves, I only have one.
My advice would just be to play the Maximum Filthy route to see what appeals to you, but if a different choice looks interesting, start a different save.
EDIT:
Best walkthrough I've seen for the Maximum Filthy route with the major scenes is this one by Mamonetti. That will get you most of the major scenes barring the sex scenes with Liam (the husband), and a couple of minor scenes on the Dylan route.
