I'm not sure if I understand this question correctly, but my take is very simple: Futanari is pure 100% fantasy, it doesn't exist in real life (I am aware of hermaphrodite/intersex but still not the same), so I don't see them the same as non-op trans-women.Do you see a difference between how these characters work in fantasy vs. how trans people could be written in a more grounded way?
