After finishing the second shot of the AV, it happens probabilistically at the Visit event.
To increase the frequency of visit events, try changing from Wardrobe to Home outfits.

Koman ending A can be achieved by making Koman as submissive as possible (see the Tips & Hints section of the game). In concrete terms: Tomie needs to be the one leading sex rather than Koman and Tomie should humiliate Koman. This ending can take quite some time to trigger, but you'll notice Koman's dialogue becoming more submissive, which indicates that you're on the right track

Just got the lottery achievement after quite a bit of grind. Definitely not worth it.

Possibly bug in the lottery text says you've won 10,000,000 but only gives 10,000. I have the save file and have taken screen shots that support this, even went to mall to buy items to ensure it wasn't just a graphical error.

My personal tips for getting this achievement after trying quite a few methods...
1) Do not waste time trying to reload on day 29/30. The ticket the old man gives you appears static so if the game tells you, you're not a winner reloading the save will not change this.
2) Do not waste time loading right before the old man touches Tomie and offers the ticket. The winning or losing ticket given, (appears to me) is decided at the start of a game and not when he gropes Tomie. Again, the game decides if lottery is LOSER or WINNER as soon as you make the game. I wasted a lot of time around day 16 (without being groped and having a ticket, then getting one before day 20 and ALWAYS lost... wasted a LOT of time with reloading old saves that would never win)...
3) I recommend starting a BRAND new game every time you fail lottery after 30 days.
4) Once you start a new game, create a save around day 1, day 5, day 10 or day 15 and just run Roaming events until you encounter the old man that gives you the ticket for $200. Old man in slums has a chance to grope Tomie and give a ticket but its not 100% and cost is still $200
5) Once you have a ticket (must be done before day 20), get 100 willpower and use Contact list and leave to cycle the days as fast as possible to day 30 to see if you got a winner.

After winning there is no additional scenes and only 10,000 and not 10mill. So at this time not worth it. The 10 mill would be neat to have, but maybe its a chance as well? Win 10k, 100k, 1mill or 10mil etc? Unknown if this is how it works.

Two achievements left... Baghead and Wonderland... Let the grind continue
