If I remember correctly, you can find out about the clinic job when you meet with the flyer girl.

You can find the hints under the Tips section, but to do it, you need to have 2 chocolate and meet BeggarA in the gorge, then meet BeggarB in sector 2 while ovulating, then you can meet the Beggar King.

you can level up your martial arts skill to lvl 13 by fighting with harassers in the slums. for a higher chance to succeed, you should first flirt with them until the chance of a successful fight goes to ~70% or something and then engage in a fight. tomie levels up every 4 or 5 successful fights iirc
